Enrollment packet checklist
What a child's record should hold on the first day, item by item. The list is Maine's, which spells it out in one place (10-148 CMR Ch. 32 §5(D)(1)); the letter after each item is its place in that list.
- Maine also keeps two records as care goes on: a same-day record of injuries and incidents, and notes on big changes in a child's appearance or behavior (items n and o). When something is missing, the record says why (§5(D)(2)).
- New Jersey adds one at enrollment: give each family the Information to Parents document and keep their signature that they received it (N.J.A.C. 3A:52-3.6(b)).
- Other states differ. Check your own rule for anything it adds.
