Child care software for Maryland centers
The parts of running a center that depend on your state, set up for Maryland instead of left for you to build.
Immunizations by your state's schedule
Each child's vaccines are checked against the CDC childhood schedule for their age, so the office sees who is due.
Maryland licensing forms, ready to send to families
10 official forms, fetched from the state's own site and kept in Farlow's form library. Pick one, mark where the answers go, and families fill it in and sign on their phone, with reminders until it comes back. Each name below opens the form on the state's own site.
Maryland State Department of Education, Office of Child Care
- Emergency Form (OCC 1214) (2 pages, checked ) archived copy
The parent lists who to call, who may pick the child up, at least one emergency contact and the child's doctor, and says which meals the child gets. If the child has a condition that could need emergency care, the back is filled in too. The whole form is updated every year.
- Health Inventory (OCC 1215) (3 pages, checked ) archived copy
Needed before a child starts. Part I is the parent's health history; Part II is the physical exam, completed by a physician or nurse practitioner (or a copy of the exam attached). It goes with the immunization certificate (MDH 896) and, under age 6, a blood lead test certificate.
- Medication Administration Authorization Form (OCC 1216) (2 pages, checked ) archived copy
For any prescription or over-the-counter medicine staff give. The prescriber fills in the medicine, dose and times and signs; the parent signs that staff may give it. Good for one year at most.
- Asthma Action Plan and Medication Administration Authorization (OCC 1216A) (3 pages, checked ) archived copy
An asthma action plan in green, yellow and red zones, filled in by the child's health care provider, with the prescriber's and the parent's authorization. Used instead of OCC 1216 for asthma medicine, for one year at most.
- Allergy and Anaphylaxis Medication Administration Authorization Plan (OCC 1216B) (2 pages, checked ) archived copy
The allergy and anaphylaxis plan: the symptoms to watch for, when to give epinephrine and call 911, signed by the prescriber and the parent. It replaces OCC 1216 for allergy medicine, for one year at most.
- Seizure/Convulsion/Epilepsy Disorder Medication Administration Authorization (OCC 1216C) (2 pages, checked ) archived copy
The seizure plan: the child's seizure types, how staff respond, any emergency medicine and care afterward, from the health care provider, with the parent's authorization. Used without OCC 1216, for one year at most.
- Individualized Treatment/Care Plan Checklist (OCC 1216D) (2 pages, checked ) archived copy
The checklist the parent and the center sign when a child with special health care needs enrolls: the written care plan, staff training, an updated OCC 1214 and OCC 1215. With a signed treatment plan that covers the medicine attached, OCC 1216 is not needed.
- Topical Basic Care Product Application Authorization (OCC 1216E) (2 pages, checked ) archived copy
The parent's permission for diaper rash cream, sunscreen or insect repellent they supply, with a log of each day's application. No health practitioner's approval and no OCC 1216 needed.
- Health History Form, Drop-In Child Care Centers (OCC 1285) (2 pages, checked ) archived copy
A short health history for drop-in centers. The parent fills it in, then reviews or initials it each time the child comes.
Maryland Department of Health, Center for Immunization
- Immunization Certificate (MDH 896) (2 pages, checked )
The Maryland Department of Health's immunization certificate: each vaccine date, signed by a medical provider, health department, school or child care provider, with a section for a medical or religious exemption.
The date by each form is the day Farlow last matched it against the state's site. A weekly check flags any link that stops working or whose file changes. States revise their forms, and the state's site has the current one.
Since October 2026 the Office of Child Care's website sends every visitor to an archived copy of its home page, so the state's links above may not open. Until it is back, each form's archived copy is the Internet Archive's saved copy of the state's own PDF, from May 2026.
