Child care software for Arizona centers
The parts of running a center that depend on your state, set up for Arizona instead of left for you to build.
Immunizations by your state's schedule
Each child's vaccines are checked against the CDC childhood schedule for their age, so the office sees who is due.
Arizona licensing forms, ready to send to families
2 official forms, fetched from the state's own site and kept in Farlow's form library. Pick one, mark where the answers go, and families fill it in and sign on their phone, with reminders until it comes back. Each name below opens the form on the state's own site.
Arizona Department of Health Services, Bureau of Child Care Licensing
- Emergency, Information and Immunization Record Card (centers) (2 pages, checked )
- Medication Consent Form (2 pages, checked )
The date by each form is the day Farlow last matched it against the state's site. A weekly check flags any link that stops working or whose file changes. States revise their forms, and the state's site has the current one.
Staff-child ratios in Arizona
From Arizona Administrative Code R9-5-404, Staff-to-children Ratios (ages: R9-5-101). In Farlow you set these bands once, and every room is counted against them as children check in and out.
| Age | Children per staff member | Largest group |
|---|---|---|
| Infants, 12 months or younger | 5 | No limit set |
| 1 year olds | 6 | No limit set |
| 2 year olds | 8 | No limit set |
| 3 year olds | 13 | No limit set |
| 4 year olds | 15 | No limit set |
| 5 year olds not in school | 20 | No limit set |
| School-age children | 20 | No limit set |
In a group of mixed ages, the youngest child's ratio and group size apply to the whole group (Ariz. Admin. Code R9-5-404(B)(1)).
The rule sets no group size. Infants may also be 2 staff to 11 and one year olds 2 to 13, and infants are kept apart from older children when six or more children are in the facility (R9-5-404(A), (C)(3)).
Read on October 29, 2026 from the state's published rule. The rule itself is what an inspector goes by.
